QUALITY IMPROVEMENT tools and resources

The International Collaborative is committed to developing tools and resources that facilitate sustainable improvement in the standard of care offered to patients approaching the end of their lives - no matter where in the world they die.   

The tools currently offered have been designed specifically to support the implementation of the 10/40 Model for Best Care for the Dying Person and were developed in consultation with the Executive Committee, the Facilitator Network, and the wider Collaborative membership.  They are available exclusively to members of the International Collaborative.

THE QUALITY IMPROVEMENT FRAMEWORK

Developed by the Facilitator Network, the Quality Improvement Framework (QIF) provides a vast range of resources that support each stage of the 10/40 Model's four-phase, ten-step implementation process.  

 

With its collection of action checklists, document templates, factsheets, project plans, learning outcomes and more, the QIF provides a comprehensive toolbox of example documents that assist healthcare professionals to implement the 10/40 Model in their own setting.   

 

Collaborative members can make use of the examples and adapt them to meet the needs of their own services.

All documents are presented in English, and also in their original language where applicable.

Clinical audit is an essential part of service improvement and can help healthcare professionals to ask important questions about how care is delivered within their organisations.  It also provides a baseline of evidence that can be used to measure progress and identify areas for ongoing improvement - a core principle of the 10/40 Model.

The toolkit offered to Collaborative members allows them to audit their clinical documentation against the core elements of the 10/40 Model, both before and after its implementation.  It automatically produces easily-interpreted results charts that can be used for reporting and comparison purposes, and comes complete with a detailed guidance document.

Care of the Dying Evaluation (CODE™)

Care of the Dying Evaluation (CODE™)

CODE™ is a 40-item self-completion post-bereavement questionnaire, based on the key components of best practice in care of the dying.  Developed and managed by Dr Catriona Mayland, it represents a user-friendly, comprehensive outcome measure for care of the dying that can be used to benchmark individual organisations and identify areas for improvement.

Implementation of the 10/40 Model will only be successful if it is supported by a robust and continuous programme of education and training.  The training required will be different for each organisation depending factors such as size, location, and the level of existing knowledge among its staff.

 

To assist organisations in designing their education programmes, we are preparing a suite of training resources that will include a competency framework and suggested syllabus content that organisations can tailor to meet their own specific needs.
